When you or a family member needs medical attention, understanding where your health plan can be used may make the experience feel less stressful. Z-2-H™ Family Med & Urgent Care Clinic provides compassionate primary care, family medicine, and urgent care at eligible Texas clinic locations.

JZ Interior Design LLC employees may be enrolled in the PPO Plan or HSA Plan. Both plans use the EHN Preferred Network, but member costs and coverage requirements differ. Verify your exact benefits and the clinic’s network participation before receiving care.

What JZ Interior Design Plans Are Available?

JZ Interior Design employees may be enrolled in one of two medical plans:

  • PPO Plan

  • HSA Plan

  • $30 copay for a primary care visit

HSA Plan

Can JZ Interior Design Employees Visit Z-2-H?

Eligible JZ Interior Design employees and their covered family members may visit Z-2-H for primary care, urgent care, preventive services, and many everyday health concerns.

The medical team can evaluate and treat concerns such as:

  • Cold and flu symptoms

  • Cough, congestion, or sore throat

  • Fever and body aches

  • Ear pain or ear infections

  • Headaches and migraines

  • Nausea, vomiting, or diarrhea

  • Urinary symptoms

  • Minor infections

  • Mild allergic reactions

  • Rashes and skin concerns

  • Minor injuries

  • Routine primary care concerns

  • Chronic condition follow-ups

  • Preventive care and physical examinations

Coverage depends on the member’s plan, current eligibility, medical needs, provider-network participation, clinic location, and the services provided.

Call 911 or visit the nearest emergency room for chest pain, severe breathing difficulty, signs of a stroke, fainting, confusion, uncontrolled bleeding, major trauma, or another life-threatening emergency.

Who Administers the JZ Interior Design Plans?

Coastal Administrative Services, also known as CAS, is the third-party administrator listed for the JZ Interior Design PPO and HSA plans.

CAS can assist members and providers with:

  • Eligibility verification

  • Medical benefit questions

  • Prior-authorization requirements

  • Claim-status questions

  • Copays, deductibles, and coinsurance

  • Provider-network participation

How Do I Verify My Benefits?

Before scheduling an appointment, contact CAS at (800) 870-1831 or SmartCare Concierge at (855) 438-2011 and ask:

  • Does Z-2-H participate with my specific JZ Interior Design plan?

  • Is my preferred clinic location in network?

  • Is the requested medical service covered?

  • What copay, deductible, or coinsurance applies?

  • Has my HSA Plan deductible been met?

  • Are laboratory tests, imaging, medications, or procedures billed separately?

Z-2-H can collect your insurance information before the appointment, but CAS or SmartCare Concierge provides the official explanation of plan benefits. Final coverage is determined when the claim is processed.

Is Prior Authorization Required?

Prior authorization depends on the medical service being requested and the requirements of the member’s plan.

Members and providers should contact CAS at (800) 870-1831 before receiving a service that may require approval. Confirm whether authorization must be obtained by the member, ordering provider, or treating provider.

Prior authorization does not guarantee claim payment. Current eligibility, medical necessity, network participation, deductible requirements, plan exclusions, and other rules may affect how the claim is processed.

What Network Does the Plan Use?

The JZ Interior Design PPO and HSA plans use the EHN Preferred Network. This network includes Baylor Scott & White Quality Alliance providers who have chosen to participate.

Not every Baylor Scott & White Quality Alliance provider participates in the EHN Preferred Network. Members should confirm that their preferred Z-2-H location and treating medical professional participate with their exact plan before receiving care.
