When you or a family member needs medical care, understanding your health plan can make the experience less stressful. Employees of Kasberg, Patrick & Associates, LP may have access to primary and urgent care through the High Plains Health Plan Option 4.

At Z-2-H™, our care team helps patients understand their coverage, prepare for their appointments, and receive convenient care for many common health concerns.

Understanding Your KPA Health Plan

According to the available plan information, the High Plains Health Plan Option 4 includes:

  • Primary care copay: $0

  • Specialist copay: $0

  • Third-party administrator: Health Plans, Inc. (HPI)

  • Network: Employers Health Network Preferred Network

A $0 copay does not necessarily mean every service is free. Laboratory testing, imaging, medications, procedures, medical supplies, or services outside the plan’s covered benefits may be billed separately.

Coverage can also depend on whether the provider participates in the EHN Preferred Network. Not every Baylor Scott & White Quality Alliance provider participates, so network status should be confirmed before receiving care.

The available plan information does not show a general specialist referral requirement. However, certain treatments, procedures, imaging services, or medications may still require prior authorization.

  • Loss of consciousness

  • A serious head injury

  • Severe allergic reaction

  • Suicidal thoughts or an immediate mental health crisis
